Hello Future China Traveler,
We are excited about the preparations taking place for our big trip this summer. Attached, you will find several forms that need to be filled out and returned to the boosters by Thursday, March 6. These forms should be mailed to JLHS PO Box address or turned in to Ms. Hurtado at the band room.
In this packet, you should find the following: - Student Travel Protection Travel Insurance Certificate Basic Plan
- Travel Insurance Summary
- Form 8-Particiapant Liability Waiver
- Form 9-Medical Authorization, Release and Waiver Agreement
- Additional Information Sheet (t-shirt sizes, special dietary needs, room assignment, etc)
- ***Form 20b-Instructions for China Visas and Application ***
- Although World Projects needs the application by May 1st, we would like to collect visa applications no later than April 18. This includes the application (China_Visa_Application_Form.pdf ), 2 passport photos, and your passport.
All Forms should be completed for EACH TRAVELER.
